Medicaid Reform
Since the federal government changed the eligibility of Medicare recipients, La Promesa (2004-2005) is one of the national initiatives that Concilio spearheads. CLDS uses its staff of culturally competent outreach coordinators to enroll Hispanic/Latino adults (65 years-of-age and above) and Hispanics with disabilities in the Medicare-Approved Discount Prescription Card. Equipped with laptops, CLDS’s staff is a mobile unit capable of covering all areas of the community. The agency has also partnered with Mountain Park Health Center and Spanish language media to increase participation in this much needed program.


In accordance with the goals of Healthy Arizona 2010, CLDS increased awareness among Hispanic/Latino older adults residing in the Enterprise Community of Phoenix regarding the importance of mental health with emphasis on depression, substance abuse prevention, with emphasis on alcohol abuse, and elderly abuse prevention as a means to promote healthy lifestyles and to prevent disease.

 

Adult Immunizations
Through Vacunas Para La Familia, the agency was able to increase participation among the elderly. This program increased their understanding of the need for, and where to obtain, immunizations that are vital to this population.
